1. The vehicle was financed, and the legal owner has the title
2. Your vehicle was titled in a different state

2. Fill out your vehicle license plate, VIN number, year, and make

If your vehicle is owned or financed by an outside company, complete Section 2.

3. Check the box in Section 3 that best applies to your situation

4. Legibly write your name, date and phone number in Section 3

5.  Sections 4-8 are only applicable if you are releasing ownership of the vehicle
